For our office to verify your coverage, we will need the following:
- Your phone number so that we can contact you
- Group, plan or policy number
- Social security number or ID of the insured
- Member and patient’s date of birth
- Name of employer
- Would you like to set up an appointment? Your family members may be covered by your insurance too and set up an appointment for them as well
